Individual psychotherapy: $325

Family therapy and/or couples therapy: $400

Payment

An updated credit card, debit card, or FSA card is kept on file in our secure electronic medical record system, and will be charged at the end of each session.


Insurance

Lifespan Therapists providers are all out-of-network. This means that we are not “paneled” with your insurance company, but if you have out-of-network mental health benefits on your plan, you may be entitled to partial reimbursement of the fee you pay your therapist out of pocket. Upon request, we will provide you with a detailed statement (a “superbill”) at the end of each month that you can submit to your insurance provider for reimbursement.

*Please note - Dr. Knafo is employed at a hospital and as a result may appear on provider lists given to you by your insurance company. Outside of her work in the hospital setting, Dr. Knafo is not paneled with any insurance companies and therefore cannot provide in-network care when working with patients privately.

Out-of-Network Benefits: Questions to Ask

Below are some questions to ask your insurance provider, if you would like some clarity on exactly how much of your sessions will be covered:

  • What are my out-of-network benefits for psychotherapy? 

  • Do I have a deductible?

  • Are there a limited number of sessions that are covered by my plan?

  • Do I need any type of prior authorization or referral? 

  • Is telehealth covered at the same rate as in-person sessions?

  • How much (percentage of “allowable amount”) will I get reimbursed per visit when I see an out-of-network mental health provider?

  • What is the “allowable amount” for individual psychotherapy (CPT code: 90834) with a psychologist (PhD) in Brooklyn, NY (Zip code: 11201)? For couples therapy or family therapy (CPT code: 90847)?
